Additional Suspected Fraud, Waste and Abuse Reporting Options: It is the policy of UNT to identify and promptly investigate any possibility of fraudulent or related dishonest activities against UNT and, when appropriate, to pursue legal remedies available under the law. Another option for reporting suspected fraud, waste and abuse in addition to reporting it to a UNT organization is directly reporting the suspected incident to the State Auditor's Office. The State Auditor's telephone number is 1-800-TX-AUDIT (892-8348) and their website is http://sao.fraud.state.tx.us.

The Role of Records Management Representatives |
Responsibilities of the Records Management Representative:
- Serve as the representative from their unit for the creation, approval and implementation of the unit’s records management procedures.
- Attend Introduction to Records Management and Final Dispositon taught by the UNT Records Management Analyst. Attend other records management training classes as suggested by the UNT Records Management Program and the Office of Institutional Compliance.
- Create and maintain an inventory of University records for their unit showing location of each record series and the Record Series Title, UNT Item Number, and Retention Period as listed on the UNT or UNT System Records Retention Schedules (RRS).
- Identify vital records held by their unit and report annually on the status of vital records to the Office of Institutional Compliance.
- Review records for assigned unit/department, disposing of those that have met their retention period on the RRS. Complete the Final Disposition Log for records that have been destroyed and immediately send original copy of log to the appropriate (see form instructions) administrator(s) for approval. Once the form has all necessary approvals, send to the UNT Records Management Analyst in the Office of Institutional Compliance.
- Communicate knowledge of UNT records management policies to other unit staff as needed and act as resource for other staff, forwarding unanswered questions to UNT Records Management Analyst or directing staff to contact the UNT Records Management Analyst directly.
- Follow the Transfer to Archives procedure for inclusion of historical records (identified in the RRS) in the University Archives.
- Notify UNT Records Management Analyst when a new RMR is appointed for unit.
RMRs should immediately notify the UNT Records Management Analyst of the following:
- Any unit records that are not listed on the RRS
- Records needing to be disposed, which are not listed on the RRS. Any record that is not listed on the RRS may not be disposed until the UNT Records Management Analyst appropriate arrangements have been made with the Texas State Library and Archives Commission.
- Records on the RRS which list their unit as the Final Repository that are no longer kept by the unit.
- Retention periods for records that do not match those on the RRS.
- Records vital to the unit that are not marked as such on the RRS. Vital records are those records that are essential to the conduct of business by the unit or the University, records that would absolutely have to be restored in case of a disaster.
- Other areas where changes might be helpful such as common names for records that should be searchable on the online RRS website.

Guide to Reporting a Concern or Complaint |
The following information was developed to assist in determining where to report a particular type of concern or complaint. This information is provided to assist UNT System employees in directing any question, concern, or complaint they may have pertaining to any type of alleged inappropriate act, conduct, or compliance with federal or state laws or UNT System policies.
The first step should always be to resolve the question, concern, or complaint using the supervisory chain-of-command. If the issue involves someone in the supervisory chain, employees should report the matter to the next higher supervisor, manager, or unit head, however, when in doubt you may always contact any of the units listed in this guidance, Internal Audit, Human Resources, Equity and Diversity’s Office of Equal Opportunity, UNT Police, the Office of Institutional Compliance, and possibly the Office of the Ombuds.
If the unit contacted is not the appropriate point-of-contact for the particular issue being raised, that unit will refer the issue to the appropriate UNT System unit for resolution. We sincerely hope that this guidance is truly beneficial in identifying the appropriate UNT unit to report your question, concern, or complaint to.

Final Records Disposition |
Final Disposition is the terminal treatment of a record, either by destruction or permanent storage in the University Archives. The disposition of records should be documented on the Final Disposition Log.
*Convenience copies are all other copies of a record that do not comprise the original record copy. Users can determine whether or not they have a record copy by locating the "final repository" field that is listed with each record series in the Records Retention Schedule. It is not required to document the disposition of a convenience copy, but users are encouraged to do so if such documentation will assist the business function of their unit(s).
A university record may not be destroyed if any litigation, claim, negotiation, audit, open records request, administrative review, or other action involving the record is initiated before the expiration of the retention period for the records series in the approved Records Retention Schedule.
**PLEASE NOTE: If the RRS has an "I" or "O" in the Archival field, you must complete the Transfer to Archives Form*

RMR Selection |
Before selecting RMR(s), please consider the following points:
- Which units report directly to you? You will need to identify a separate RMR for each unit.
- What is the size of the unit(s)? What is the volume of records maintained in the unit? Larger units will be required to identify more than one RMR so that all functional areas of the unit are properly addressed. Administrators should consider the records within each functional area of the larger unit. It is advisable to identify one RMR per functional area.
- Which staff handle the maintenance and operation of unit records on a regular basis? RMRs will receive training and guidance on records management policies and procedures. RMRs will develop a skill set that will compliment their existing knowledge of unit processes.
- What is the reporting structure of the unit? RMRs should have access to unit administration and management - but do not have to be a part of the management team. RMRs are the point of contact for the UNT Records Management Program and are involved in the approval process for disposing records of the unit.
RMR/Alternate RMR Identification & Training Registration Process:
- Unit Administrator/Head receives request from UNT Records Management Analyst to identify RMRs/Alt RMRs.
- Unit Administrator/Head determines the appropriate number of RMRs/Alt RMRs for their unit. The UNT Records Management Analyst assist units in determining the correct number of RMRs.
- The Unit Administrator/Head notifies the individuals that were selected to become RMRs/Alt RMRs. The Unit Administrator/Head or the employees complete the RMR/Alternate RMR Identification Form [Online - EUID login required].
- RMRs/Alt RMRs register for required Records Management Training.

Records Management Representatives |
Records Management Representatives (RMRs) and Alternate RMRs
As part of the Records Management Program, the Office of Institutional Compliance has developed records management training and guidance for the UNT Denton Campus, UNT Dallas Campus and UNT System Offices. In an on-going effort to equip each unit with the tools necessary to appropriately maintain and dispose of University records, Institutional Compliance is asking that each unit identify individuals who will become Records Management Representatives. All RMRs will receive specialized training, information and guidance which will allow them to be the records management resource for their unit.
